Recipient Request Form

There are two options to receive your scholarship funds:

  1. Complete the Recipient Request Form for payment of scholarship

You will need to following in order to submit the request:

  • Copy of the thank you note you sent to the Scholarship Donor.
  • Proof of registration  (Proof of registration may be satisfied by a copy of your class schedule if it shows the number of credits taken.)
  • Cost of schooling (tuition and housing, if applicable),  this is best shown with a bill from your school; and
  • A mailing address for your school’s billing office; along with your STUDENT ID number.  The school’s bill paying mailing address is very IMPORTANT–because your check will go directly to your school’s bursars’ office and your student ID number is crucial to make sure our payment is credited properly to your account!

*Scholarship payments begin after July 1st of each year. If your scholarship is paid in two installments, a second request form should be completed after your second semester enrollment information is available.

2. To claim your funds, send proof of registration along with your completed Recipient Request Form for Payment of Scholarship to BCSC, P.O. Box 380, Baraboo, WI  53913.  This form will be given to you on Honors Night.
